FAMILY SERVICE COORDINATOR
Valley Manor Long Term Care is
recruiting for a Full time Resident &
Family Service Co-ordinator
PRIMARY DUTIES:
Administrative support for the nursing department.
Creates and maintains Admission packages in compliance with the FLTCH Act and
inspection protocols;
Coordinate the Admission Process and resident’s accounts.
Provide facility tours
Contribute to the efficient operation of the Nursing and Administration departments
and strives to complete all duties in the spirit of Valley Manor’s Mission, Vision and
Values.
Good understanding of the Long-Term Care Act and Regulations
Must be able to multi-task in a very fast-paced environment.
Fosters positive working relationship with discharge planners and Home & Community
Care.
Coordinate resident intake, transfer, and discharge in consultation with stakeholders;
Acts as a support to residents and their families
Coordinates the eRRISSA on admission as required and assists in the annual rate
reduction in coordination with the Accounting Clerk
Organizes and maintains confidential files relating to Nursing Administration and
Human Resources.
Privacy Officer for record management including electronic files
Prepare nursing schedules in collaboration with the Director of Care
Implement payroll for nursing department.
Assists with inventory of Medical and Nursing Supplies and Ordering
.
QUALIFICATIONS:
Diploma from a recognized college related to Administration, social
services, healthcare or gerontology from an accredited community college.
Minimum of 2 years of experience in an administrative position, preferably
Long-Term Care, or, at a minimum, in a health care setting experience in
managing a comparable resident and family service
Experience taking minutes and creating reports
Proficient with computers, payroll systems, point click (word, excel,
spreadsheet, database)
Proven interpersonal abilities with excellent verbal, written and
interpersonal communication skills
Must be able to multi-task in a very fast-paced environment.
Knowledge of union contracts is considered an asset
Salary range $50,000-$53,059.50
