The Direct Care Co-ordinator is accountable to the Director of Care and the residents for the day-to-day co-ordination and supervision of the nursing department and resident care services. Through effective co-ordination of all resources in the nursing department and development of a strong nursing team, the DCC will ensure the delivery of optimal direct care, mandatory programs and service, based on legislative requirements and Valley Manor’s Code of conduct and standards. This role includes a variety of shifts which shall include days, evenings, nights & every second weekend This position remains part of the ONA Bargaining Unit and will remain available as long as the Ministry continues to fund this position.

Qualifications:

  • Current Certificate of Competence from the College of Nurses of Ontario as a Registered Nurse,
  • 2 years’ experience in Long Term Care or equivalent clinical skills and knowledge of gerontological nursing practice,
  • Post RN education and experience in gerontology or related field preferred,
  • Supervisory or management experience is an asset,
  • Palliative Care specialization is an asset,
  • Requires excellent computer skills including proficiency in PCC/POC/eMAR & Word,
  • Must have working knowledge of the Fixing Long Term Care Act 2021.

Duties

  • Co-ordinates, demonstrates, directs and monitors the effectiveness of all front-line care within the nursing department;
  • Delegates responsibility to nursing staff based on clearly defined lines of authority and communication;
  • Participates in an effective quality and risk management system to ensure legislative regulations and standards are met and to improve the direct care for a better resident experience;
  • Participates in corporate and external committees as requested;
  • Participates and works closely with the Director of Care and front-line staff to ensure optimum performance measures are met as per MOHLTC standards in an effective process in accordance with Valley Manor’s standards for all employees within the nursing department;
  • In collaboration with the NCC and the DOC, assists in the development, monitoring and evaluation of staffing assignments and work schedules within the nursing department.
  • Identifies nursing department learning needs and makes provisions for appropriate education and continuing staff development;
  • Fosters positive working relationships with the leadership team, union colleagues, families and employee representatives;
  • Ensures that care provided is documented as required in a timely manner;
    • Contributes to the development of job routines for all nursing personnel to improve the direct care resident experience;
    • Liaise with medical staff, nurse specialists, and consultants and supports effective clinical care for all residents to improve the direct care resident experience;
    • Replace front line nurses, charge nurse when required to improve the direct care resident experience;
  • Completes all mandatory education and emergency code training;
  • Being aware of fire plan and disaster plan within Valley Manor;
  • Any other duties.
