DESCRIPTION:

MINIMUM REQUIREMENTS:
- Current registration with the College of Nurses of Ontario
- Education qualifications must be from an Accredited post-secondary institution

DUTIES:
- Reporting to the CEO, collaborate and liaise with the Medical Director, Director of Care and interdisciplinary team in care and delivery
- Assess, diagnose, and evaluate acute and chronic illness and other changes in resident’s conditions, prescribe treatments, diagnostics and follow up care.
- Support and advocate for residents’ best interests while engaging with the resident / family / substitute decision maker (SDM) regarding Care.
- Participate in resident care conferences.
- Provide clinical leadership to the LTC team members centered around the resident care needs which can include resident move in/out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
- Participate in identifying, analyzing and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents and families.
- Promote knowledge development and mentorship of clinical staff by integrating best practices in resident care.
- Assist with the development and delivery of education to staff, residents and families.
- Document clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely manner.
- Participate in creating an organizational environment that supports safety and quality of resident care and life, collaborative practice, and professional growth.
- Participate in Quality Committee meetings; support CQI projects and initiatives.
- Participate in providing after hours and on-call coverage

STARTING WAGE: $52.00–$62.00 per hour (Transparency statement: Compensation for this role is supported through Ministry funding. Placement within the salary range is based on role requirements and applicable policy)
Pay Transparency Notice: Where applicable under the (ONA) collective agreement, newly hired nursing employees will be placed at the entry rate of pay at the time of hire. An employee may be eligible for a rate adjustment based on prior relevant nursing experience, provided that: Verifiable proof of qualifying experience is submitted within three (3) months of the date of hire; and the experience meets the criteria for recognition as set out in the collective agreement.
