Gwen Brown (Board Member) - Valley Manor Inc.

Gwen Brown, Board Chair

I was very pleased to join the Valley Manor Board of Directors in January, 2024.  My family have had a cottage in the area for over 65 years and I became a permanent resident in 1990.  The beauty of this community has been a draw since early childhood.

I have had a varied career in health care including front line nursing and management roles across various sectors.  This cross-continuum experience has provided me with a comprehensive understanding of the Ontario healthcare system including long term care.  Within the last 30 years, my roles have focused on knowledge translation, transformational change, integration initiatives, implementation of best practices and the development of strategic partnerships.  Leading interdisciplinary, provincial project groups has given me insight into working with dynamic, diverse partnerships to achieve shared goals.  I have also had the pleasure of leading community consultations and patient advisory groups enabling a greater understanding of and response to identified needs and successes.

My education has included Bachelor degrees in Arts and Education and a Masters in Integrated Studies.

Outside of the healthcare field, I have had an ongoing interest in supporting the community I live in through various volunteer positions including membership on the Executive for my lake association, the Killaloe Curling Club and, when my children were young, the local school parent advisory council and the Killaloe Library Board.  I very much look forward to working with, learning from and contributing to the Valley Manor Board of Directors.

Cathy Borutski (Board Member) - Valley Manor Inc.Cathy Borutski, Board Vic-Chair

I eagerly joined the Valley Manor Board of Directors in June 2024. Having been in the healthcare industry for my entire working career I began in acute care and moved into Long-Term Care and Retirement. The last 28 years have been working in senior care, much of that time as a General Manager.

There is nothing more satisfying than helping our seniors settle and thrive in their new living environment as this transition can be difficult to both the senior themselves as well as their loved ones.

In May of 2023, my husband and I moved to the Ottawa Valley from Southern Ontario to be closer to family and the transition has been amazing.  We have two adult children Joel and Kendra.

In my spare time I enjoy gardening, baking, and running. I am honoured to be part of the Board of Directors at the Valley Manor and look forward to supporting the homes mission, vision, and values.

Liz Polatynski (Board Member) - Valley Manor Inc.

Liz Polatynski, Secretary/Treasurer

I joined the Board in February. I have been in the nursing profession for 50 years starting as a daughter of St Michael's Hospital in Toronto where I developed my interest and commitment to serving vulnerable communities including the urban poor, inner city population, underhoused and homeless and those with persistent serious mental health issues including addictions.

I started as a front line staff nurse and worked for the next 35 years in various positions including critical care, education, and Clinical Leader Manager. I developed an interest in the role of Nurse Practitioner as a means of extending service to vulnerable populations; I have a Masters Degree in Advanced Clinical Nursing Practice; I have practiced for the last 17 years in primary care and as a hospitalist at CAMH for 10 years.

Having retired and moved to Barry s Bay to the family cottage, I volunteered at the Madawaska Palliative Care Hospice and eventually was hired as an NP at the Madawaska Valley Family Health Team. It was my profound honour and pleasure to establish the NP practice with the support of the Board of Directors in order to provide access to care for the members of the community who did not have a primary care provider. Our aim was to provide service to the most vulnerable members of the community . I am currently a locum covering the NP practice.

It is my great pleasure to have been selected as a member of the Board of Directors to assist with the governance of VM; I believe in the strategic vision of caring for the caregivers and look forward to supporting Valley Manor as they step forward into the future.

Continuous Quality Improvement - CQI

A set of activities that are carried out to set standards used to monitor and improve performance so that the care and services provided will satisfy stated or implied needs. These processes don’t “assure” quality; they are tools used to assist with the management of quality.

The CQI Team -

The CQI team at Valley Manor provides the framework for the development and implementation of our integrated Quality Management system.

We strive to provide and implement appropriate quality management systems and processes to ensure the delivery of the highest quality care and service to our residents.

The Valley Manor CQI team continues to provide the framework for continuous improvement  which improves the  resident satisfaction a at Valley Manor and with our community stakeholders.  We must share the responsibility for the delivery of high quality programs and services and for continual improvement. Ultimate responsibility for quality and risk management activities rests with the CQI team however, all stakeholders including staff, volunteers, board members, and third party service vendors have a role to play in improving quality and managing risk. Direction for the Quality and Risk Management program is provided by CQI team.

Our CQI team is  committed to continuously improving the quality of care and service that we provide. Quality means meeting the needs and expectations of our residents and by extension, their families and/or support systems. Each of us has a responsibility to ensure quality for the residents that live at  Valley Manor.

Quality is improved through a continuous process of self-assessment with targets and standards, reviewing and evaluating progress and implementing quality improvement and other action plans. The policies, procedures, implementing processes and monitoring mechanisms are made known to our residents, families and stakeholders with the aim of involving everyone in the development of our culture. In order to develop this quality culture we use a range of ways to consult and inform, and aim to continuously improve effective communications.

Quality Improvement Plan (QIP)

The Excellent Care for All Act (ECFAA) requires that every year, health care organizations including Long-Term Care develop an annual Quality Improvement Plan (QIP) for the following fiscal year and make that plan available to the public.

The QIP is an organization-owned plan that establishes a platform for quality improvement that can be used to harmonize efforts to improve quality of care across the health care system. While each organization owns, develops, and maintains their QIP, organizations share in a common agenda of improving the patient/resident experience that is guided by provincial priorities for high quality care as highlighted in Ontario’s Action Plan for Health Care.

Our current QIP is accessible right here: https://qipnavigator.hqontario.ca/

On Tuesday, October 1st at 10:30 am Valley Manor LTC home performed their triennial full evacuation drill under the observation of the Madawaska Valley Fire Chief, Corwin Quade. At the staff debrief after the event, he advised the staff that “You all did quite well! You all did a great job, so obviously you know what you’re doing.” Valley Manor’s CEO, Trisha DesLaurier also commended the staff on a job well done stating “A huge thank you to the entire team for your participation in the full evacuation exercise at Valley Manor today! It was completed in record time of 24:48 minutes. The last full evacuation drill was completed in 32 minutes. A job well done by all; including the residents!!”

Why conduct this drill?

Exercises or drills provide several important benefits to any organization that uses them to prepare for its emergency response. Some drills are conducted monthly, yearly and triennially as per the Ministry of Long-Term Care’s - Fixing Long-Term Car Act, 2021. Conducting this drill helps the home increase their overall preparedness in the event of a real evacuation. In particular, it:

• Provides the home a way to determine its readiness to respond to a crisis or disaster.

• Clarifies gaps or problems with existing policies and plans.

• Helps administration and staff understand their roles during a disaster.

• Serves as a training tool.

• Helps identify needs for other resources, and

• Serves as a tool for modifying and improving existing plans based on the lessons learned during the drill.

Present: Left to Right

Board members Annie Marsh, Bill Rodnick, Jill Vermaire, Director of HR and Finance Stacey McDonald, Honourable Minister of Long-Term Care Paul Calandra, CEO Trisha DesLaurier, MPP John Yakabuski and Director of Care Gail Yantha

Honourable Minister of Long-term Care Paul Calandra attended Valley Manor on Monday, June 12th to review Valley Manor’s final stages of their redevelopment plans with John Yakabuski MPP, the Valley Manor Executive Team and the Board of Directors.

Valley Manor was eager to showcase the great redevelopment work that has been completed to date, despite the fact that the home is situated in a small rural community and faces additional challenges being a not for profit home as compared to larger communities.

Minister Calandra and MPP John Yakabuski reviewed the redevelopment plans and toured the facility with the team. Minister Calandra was very complementary to the redevelopment and fundraising work that has been completed so far, despite the challenges of the pandemic and being in a rural location which presents an additional layer of challenges to complete the journey.

The meeting was a part of Valley Manor’s advocacy efforts for an individual funding model for small rural homes with limited fundraising capacity.

The new LTC funding model is such a game-changer: jump-starting the Valley Manor redevelopment project by 4.3 million plus an upfront planning grant based on a percentage of the total cost of the project.

This new Funding Model is extremely beneficial for Valley Manor’s development project as there is new funding based on the demographics of the home and moves away from a one-size fits all approach which made the project more complex and challenging.

Barry’s Bay is defined as a “Rural centre” with a core population of less than 10,000 people, therefore,  Valley Manor  will be funded in that category.

Valley Manor does qualify for a Grant  based on a  percentage of total project costs.
This grant will be provided up front to expedite the redevelopment process.

Additionally, Valley Manor is constructing the new home based on a 60% Basic room, 40% private rooms model to meet the needs of the community that it serves.    The Ministry will  provide increased funding for the increased basic room percentages.

This new funding announcement is a victory for Valley Manor  and the next step is to sit down with  Infrastructure Ontario to review the new numbers  working the IO Long Term Care loan plan.

The pandemic has directed the Ministry’s  attention to long term care and the  Ministry is responding swiftly to get these LTC homes built for  the  senior population now and in the future.

Acknowledging that our most vulnerable seniors who have worked all of their lives raising their families and contributing to the success of our province should be respected and acknowledged in their golden years. They were once us and they deserve to reap the rewards of their contributions to our province with dignity.

More often than not, you have experienced one of our volunteers, caregivers, or staff  go the extra mile to ensure either you or a loved one has just what you need.

Perhaps the receptionist has gone out of their way to help you with a phone call, the administration office assisted you to get the information you need quickly, a volunteer helped you at the door and gave you that smile you needed, or the great care staff at our healthcare organizations provided you or a family member with extra special care when you needed it most.

We are so very proud of everyone who makes up our community's wonderful healthcare team!

Mission

To create an opportunity for the donors of th Madawaska Valley to advance healthcare in the community by ensuring support for equipment and facility needs through the effort of one fundraising organization that will inclusively fundraise based on a long term plan for the community’s non-profit healthcare needs.

In this way, our donors are making an overall impact on all the healthcare organizations in their community, knowing that these organizations have taken the communities needs into account when planning for what we will need in the long term.

Who do we raise Funds for?

In accordance with an overall long range plan, the SFVH Foundation raises, manages and distributes funds to:

  • St Francis Memorial Hospital (including Rainbow Valley)
  • Madawaska Valley Hospice Palliative Care
  • Valley Manor Nursing Home

What do we raise money for?

The 3 non-profit Healthcare Organizations support partnerships around the current needs of patients, putting the needs of the community ahead of their independent needs and identifying their capital equipment needs as it relates to long term planning.

Based on the community’s capacity to give, fundraising strategies are developed by the SFVH Foundation to meet these with a projected 5 year plan.

Campaigns and Designated Funds

When you give to the SFVH Foundation, you are actually supporting all three healthcare organizations (SFMH, Valley Manor and Hospice) because the fundraising dollars go back to these organizations at the end of the year to support their equipment needs based all based on an overall plan that reflect the community’s needs. However and as always, the Foundation will continue to respect the wishes of its donors to designate a gift of support if requested.
