Duties

Experience in Activities in Long Term Care is an asset
Computer literacy (Word perfect & Microsoft Publisher- includes Rai-Mds, POC and PCC) Recording and documentation skills.
Ability to learn, demonstrate and utilize a variety of communication techniques, media and skills in dealing with the elderly and disabled in recreational pursuits.
Knowledge and courses in behaviors, Montessori, Gentle Persuasive Approach, etc. an asset.

REGISTERED DIETITIAN

Valley Manor Long Term Care is recruiting for a part-time Registered Dietitian to work onsite.

This position requires 45 hours per month of dietitian support based on the Fixing Long Term Care Act and Regulations.

Education & Experience

  • Member in good standing of the College of Dietitians of Ontario
  • Must have liability insurance, TB testing and vulnerable sector police check
  • Membership in Dietitians of Canada – Gerontology Network an asset
  • An active member of CSNM is considered an asset
  • Experience in Long Term Care is an asset
  • Ability to work in a team and independently
  • Good verbal and written communication skills
  • Excellent organizational and time management skills
  • Demonstrates an ability to problem solve
  • Computer Literacy - knowledge of computerized electronic medical records (EMR), Point Click Care programs an asset

Responsibilities

  • Identify and monitor level of nutrition and hydration risk for all Residents.
  • Complete scheduled RAI-MDS assessments; assess nutritional status for all Residents upon admission, annually and when significant changes in status occur.
  • Prioritize and respond to dietary referrals in a timely manner.
  • Develop, evaluate and modify nutritional care plans for all Residents; liaise with Residents, family members and/or substitute decision makers and the interdisciplinary team when implementing dietary interventions.
  • Conduct swallowing assessments, as needed.
  • Provide nutrition counselling and education to Residents and family members and/or substitute decision makers, as needed, in line with supporting Residents’ nutrition care.
  • Collaborate with the Food and Nutrition Manager to implement a pleasurable dining experience for Residents.
  • Participate in Care Conferences and committee meetings as needed.
  • Review and approve seasonal menus; develop individualized menus for Residents, as needed.
  • Provide education and training to dietary and nursing staff to support home-specific policies and procedures, as needed.
  • Participate in home-specific continuous quality improvement initiatives, as needed.

Position Available:-37.5 hours weekly

Performance Requirements: To perform all duties in activity and related therapies for each and every resident of Valley Manor.  To promote and provide a balanced and varied programs of activities that interest and engage residents in the fulfillment of their individual needs and quality of life according to MOH - LTC standards.
Includes: computer work, constant walking, turning, bending, dancing, climbing, lifting, stretching, ladder work, physical labor, equipment moving and handling, safe practice of all of the above.
Qualifications: 
    ~  Post-Secondary diploma or degree in recreation and leisure, therapeutic recreation,

Assist with the development and delivery of education to staff, residents and families.
Document clinical data, assessment findings, diagnoses, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely manner.
Participate in creating an organizational environment that supports safety and quality of resident care and life, collaborative practice, and professional growth.
Participate in Quality Committee meetings; support CQI projects and initiatives.
Participate in providing after hours and on-call coverage
